Position DescriptionEmployees in this classification provide patient care management to an assigned clinic in the areas of clerical and clinical duties. In the clinical setting, the employee receives direct supervision from a physician or nursing supervisor in either oral or written form. In the office setting, the employee receives direct supervision from the department administrator. Work is performed under usual office and clinic conditions, and may be stressful when working under emergency conditions.
Major/Essential Functions- Obtains and records vital signs, time in/date, weight, medication profile, immunization record, allergies, chief complaint, interventions, patient responses, and signatures/initials where appropriate. This includes administering medications, performing treatments (e.g., wound care, injections), and monitoring vital signs.
- Assisting with patient care plans: Contribute to the development and implementation of care plans under the guidance of LVNs, RNs or physicians.
- Educating patients and families: Help patients and their families understand their health conditions, treatment plans, and care instructions
- Communicating with Healthcare Team: Report patient status, changes, and concerns to LVNs, RNs and Physicians.
- Collaborating with health care professionals: Works with other health care professionals to provide comprehensive patient care.
- Gathers data relevant to the patient's individual needs and age group. Elicits important information concerning the patient's progress, gathers data regarding patient needs while providing direct patient care, and documents nursing observations according to hospital charting standards.
- Reports symptoms, reactions and progress of the patient to the clinical supervisor and/or charge nurse and appropriate action has been taken. Pursues the proper chain of command to attain a satisfactory solution. Initiates changes to improve patient care; discusses changes with the doctor when appropriate.
- Utilizes educational opportunities within the hospital and other avenues to maintain clinical expertise to promote personal growth and development. Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
- Conducts individual patient assessment and interview, and prioritizes the data collected to decide the medical problem/condition and documents in a chart for the physician.
- Reviews physicians' orders, lab requests, or follow-up needs with patients. Follows up with patients on lab results, including the scheduling of any other tests. Documents services performed and patient assessment findings.
- Sterilizes or cleans equipment and supplies. Cleans rooms according to the protocol after each patient and at the end of the day. Maintains supplies, including stocking and ordering new supplies and disposing of expired or damaged supplies.
- Helps with administrative tasks such as answering patients' calls, EMR Portal Messages, e-mails, and scheduling procedures while maintaining patient confidentiality. Responds to patient inquiries by evaluating and responding to the patient's problems and questions. Documents services performed and patient assessment findings.
- Coordinates procedure scheduling/procedure prior-authorizations, Botox medication prior-authorizations and Rx refills/Rx prior-authorizations for patients through insurance and other physician offices.
- Aides individual teaching plans for each patient, based upon a plan of care by LVNs, RNs or physicians. Ensures that care reflects the goals of patient outcome activities.
- Utilizes non-patient care time to help other nurse members or physicians with tasks, patient paperwork, refills, etc.
- Maintains a current CPR (BLS) card and current certification/license.
Required QualificationsA minimum of a High School diploma or equivalent. Graduation from an approved certified program for assistants in the area of required specialty. Certification by an approved accredited certifying agency in the specialty in which the particular work will be performed.
