SUMMARY OF POSITION

The County of Monterey Health Department is seeking motivated applicants for two full-time, permanent Community Service Aide II positions in the Public Health Bureau’s Enhanced Care Management (ECM), and Maternal, Child, Adolescent Health (MCAH) programs.

The ECM Program serves clients who are experiencing homelessness, have chronic medical conditions, are high utilizers of the emergency department and/or hospitalizations, and/or have severe mental health diagnosis. The CSA II’s in the ECM program will accompany staff to appointments with clients, monitor client’s progress toward goals, report pertinent information to the care team, provide basic health education, assist with education on community resources and linkage to services, assist clients in keeping appointments, and perform basic data entry, including documenting activities/discussions with clients on the electronic medical record. The CSA II’s will conduct outreach to new and potential clients and provide a basic explanation of services as well as converse about the benefits of the ECM Program. The CSA will also support the program by providing language interpretation to ensure appropriate service delivery that is both culturally sensitive and patient centered.

The Maternal, Child, Adolescent Health (MCAH) Home Visiting Program is committed to serving women, children, teens, and their families in Monterey County by improving access to comprehensive, quality health care, and focusing on prevention and early intervention strategies. The program partners individuals with a registered nurse case manager who visits with clients in their home and/or in the community and works with them to achieve their health goals. The CSA II will support the program by providing language interpretation to best meet client needs and ensure appropriate service delivery that is both culturally sensitive and patient-centered, perform basic explanation of services to new and/or potential new clients, assist the team with linkages to services, and conduct outreach in the community.

The Eligible List established by this recruitment process will be used to fill current and future vacancies, both regular and temporary, as they arise.

This classification is part of a flexible series. Incumbents appointed at the lower level of the career series may be promoted up to the higher level of the career series subject to their meeting the employment standards for the higher class and a recommendation for their promotion by the appointing authority in accordance with the Flexible Staffing Program and Guidelines.
