Description: Jefferson County Human Services has established a new division to coordinate case management, care planning, and make referrals to other resources for persons eligible for any of Colorado’s Medicaid Waivers under Health First Colorado (Colorado's Medicaid program). The new division was created to comply with Federal law that requires case management be provided by an agency that is separate from the agency that provides direct care. The intent is to simplify access for individuals and families and create consistency with services provided across the state. This redesign has created a unique opportunity for someone with strong leadership skills to guide new and seasoned employees in navigating this transition while ensuring the services and needs of our clients and the community are met.

Case Management Associates responsible for supporting client-facing work of this division.
Case Management Associates perform substantial support work providing services related to case initiation and caseload management within the Case Management Agency. The position is responsible for screening referrals, assisting leadership with triaging referrals, and ensuring effective communication and cooperation between our Division, internal partners, and external partners
We are hiring Case Management Associates for the following programs :
IDD (Intellectual and Developmental Disabilities) Program - The teams in this program are responsible for the long-term case management for clients with Intellectual and Developmental Disabilities and there families.
Non-IDD Program - The teams in this program are responsible for the long-term case management for clients experiencing needs related to physical disability, chronic illnesses, and aging.

Schedule : The county operates primarily on a 4 day/10-hour work week. (Monday-Thursday); Human Services works a mix of 4 day/10-hour or 5 days a week (Monday-Friday). This position is hybrid, with remote work and in-office work as determined by team and agency needs.
Hiring Range:$26.00 - $27.21 USD Hourly Compensation will be determined based on education, experience, and skills.

RESPONSIBILITIES Case Management Associates have the following responsibilities:
- Receives and processes new applications and referrals and related supporting documents within state mandated time frames.
- Assisting with referrals of pending HCBS cases and gathers and documents information on financial eligibility status from CBMS.
- Researches internal workflow databases for history and status of identified parties in applications and records information into the state data system.
- Conducts complete case reviews, determines appropriate case routing, and transfers cases to Case Managers.
- Conducts research utilizing various computer-based applications including Care and Case Management (CCM) System, CBMS, and GenApp.
- Assists with reviewing changes or additions to procedures and policies to confirm the workflow and identify potential issues.
- Documents and processes referrals and billable units in the CCM within state mandated timeframes to support records maintenance and reporting.
- Assists customers in completing the application process and in navigating other Human Services programs and community resources to ensure families are receiving needed assistance and services. Refers customers to appropriate resources.
- Communicates internally and externally to ensures effective collaboration and cooperation between our Division, internal partners, and external partners.
- Provides orientation coordination for new employees of the Case Management Agency.
- Other duties and responsibilities as assigned.

Minimum Qualifications:
- High school diploma or equivalentand;
- 1 year of work-related experience.
- Or an equivalent combination of education and experience.
